I'd say Reage as credited, since the true identity of Reage was hidden for so long.  That's certainly what most people would search under.

From the Rules:

"Use the "As Credited" field where the person's name differs from the credited name."

Proper credit in DVDP = Dominque Aury credited as Pauline Reage.

I think you are right. The author of the novel has always (until a few years ago) been known as Pauline Reage, her nome de plume, while her true identity was unknown.
The movie dates back to 1975.

It seems to me this is similar to the case of an actor who always used a stage name, never the real name.
For instance, would you enter "Norma Jeane Mortenson credited as Marilyn Monroe"? I don't think so.
